This Challenge is designed to spur a collaborative approach to identifying health IT solutions to support the integration of health care and social services. The need for seamless integration across health and social care sectors is growing, and with it, the demand for interoperable, scalable infrastructure to improve efficiency. ACL challenges a team of key stakeholders, including state and community leaders in the aging and disability network, health care systems, health plans, and health IT vendors, with working collaboratively on optimizing interoperable and scalable technology solutions.
