ACL Announces 2021 National Chronic Disease Self-Management Education (CDSME) Resource Center Award

ACL is pleased to announce the recent award of the 2021 National CDSME Resource Center to the National Council on Aging (NCOA), Inc. The cooperative agreement awards an anticipated $10 million during the five-year project period, from August 1, 2021 through July 31, 2026.

The National CDSME Resource Center will continue to provide technical assistance, education, and resources for the national CDSME network to increase the number of older adults and adults with disabilities who participate in evidence-based CDSME and self-management support programs, especially in underserved populations and communities. The Center will also support Community Integrated Health Networks (CIHNs) within states, regions, and tribes that provide evidence-based CDSME and self-management support programs, and related services that address social and behavioral determinants of health.

To achieve this, the National CDSME Resource Center will:

  • Provide leadership and guidance to the aging and disability networks through comprehensive technical assistance to enhance the reach and sustainability of CDSME and self-management support programs, including CIHNs.
  • Serve as a national clearinghouse to identify, develop, and share education and resources around healthy living and chronic disease self-management for older adults and professionals who serve them.
  • Maintain a national CDSME database to track program progress, and use program data to advance the field.
  • Identify new health promotion and disease prevention programs to include as approved ACL evidence-based programs.
  • Host special events including the Older Adult Mental Health Awareness Day Symposium and a national conference.
