Self-Direction for Dually Eligible Individuals Utilizing Long-term Services and Supports (LTSS)

Self-direction, or consumer-direction, allows consumers to direct and manage their personal care services. It is based on the idea that people who receive LTSS know their needs best. Self-direction promotes personal choice and control over who provides services and how services are provided. Evidence shows that participants in self-directed LTSS programs are more satisfied and have better outcomes than people receiving agency-based services.

Offering consumers the choice to direct their own services can present challenges for health plans. This webinar will discuss the importance of care coordinators, person-centered care planning, and assisting members with management tasks required for self-direction for plans seeking to support consumers in directing their own care.

Individuals with disabilities dually eligible for Medicare and Medicaid are at higher risk for poor health outcomes than individuals without disabilities. This event is a platform for questions on how health plans and providers can improve health outcomes by addressing gaps in care coordination.
