Community health workers (CHWs) play a valuable role in addressing the social determinants of health for older adults by building community capacity and trusting relationships for education, outreach, counseling, support, and advocacy. Join this webinar to learn how two organizations have engaged CHWs in implementing evidence-based chronic disease self-management education and falls prevention programs. Speakers will discuss a chronic care management program that has been jointly developed and implemented by a federally qualified health center and university, as well as a community health worker online training program focused on falls prevention and chronic disease self-management.
