View this email in your browser. April 18, 2024 What's New Webinar Series on Rural Maternal Health Continues – Tuesday, April 23 at 3:00 pm Eastern. Registration is open for a four-part webinar series presented by the Federal Office of Rural Health Policy (FORHP), in collaboration with the Rural Health Information Hub. The fourth session highlights resources available for obstetric readiness in rural facilities without birth units. Attendees will learn about resources available to be ready and respond to patients' needs. Recordings and transcripts of previous sessions are also available. Rural-Urban Disparities in SUD Screening and Treatment. Researchers assessed disparities in the availability of hospital-based transitional opioid programs for substance use disorder (SUD) in urban and rural settings. They discovered that in urban hospitals 73.2 percent conducted SUD screenings in the inpatient setting, and 78.6 percent did so in the emergency department. By comparison, only 54.5 percent of noncore rural hospitals provided SUD screenings in inpatient settings and 65.5 percent did so in rural emergency departments. The study also revealed a significant disparity in the availability of medications for opioid use disorder and addiction consult services, with rural hospitals having far fewer resources. The Government Accountability Office Issues a New Report on Health Centers. The federal agency that examines how taxpayer dollars are spent reviewed grant funding, revenue, and efforts to meet certain access-to-care requirements for health centers, a key part of the nation's health care system for nearly 60 years. In 2022, about 1,400 health centers operated almost 15,000 service sites and provided care to more than 30 million people in the United States, regardless of their ability to pay. According to the report, this figure includes more than 9 million rural residents and over 1 million people experiencing homelessness. Health centers rely on revenue from a variety of public and private sources, including grants awarded by the Health Resources and Services Administration (HRSA) through its Health Center Program. Among other things, HRSA requires health centers to have locations and hours that are responsive to patient needs, and to have procedures to handle medical emergencies that arise after hours. GAO found that health centers' total revenue rose from about $28.7 billion in 2018 to $42.9 billion in 2022. The largest single source of revenue was Medicaid, accounting for over one-third of total revenue each year, followed by grants, including those provided by HRSA. OMB Seeks More Public Participation in Government Decision-Making – Comment by May 17. To improve public perception and trust in government, the Office of Management and Budget (OMB) is looking for recommendations on ways to improve public participation in a wide range of functions, regulations, policies, and programs across federal agencies. OMB welcomes input from a diverse array of stakeholders in the public, private, advocacy, non-for-profit, and philanthropic sectors, including State, local, Tribal, and territorial governments as they work to develop a government-wide framework for better engagement with the public. For the purposes of this Request for Information, the term "underserved communities" refers to populations as well as geographic communities that have been systematically denied the opportunity to participate fully in aspects of economic, social, and civil life, including those living in rural areas. Ongoing: Requirements for Buprenorphine Prescribing. As announced by the Substance Abuse and Mental Health Services Administration in January 2023, clinicians no longer need a federal waiver to prescribe buprenorphine for treatment of opioid use disorder. Clinicians are still required to register with the federal Drug Enforcement Agency (DEA) to prescribe controlled medications. On June 27, 2023, the DEA began to require that registration applicants – both new and renewing – affirm they have completed a new, one-time, eight-hour training. Exceptions for the new training requirement are practitioners who are board certified in addiction medicine or addiction psychiatry, and those who graduated from a medical, dental, physician assistant, or advanced practice nursing school in the U.S. within five years of June 27, 2023. Rural Health Clinics (RHCs) still have the opportunity to apply for a $3,000 payment on behalf of each provider who trained between January 1, 2019 and December 29, 2022 (when Congress eliminated the waiver requirement). COVID-19 Resources Learn About Long COVID & Fatiguing Illness – Thursday, May 9 at 3:00 pm Eastern. The Centers for Disease Control and Prevention funds this monthly webinar-style ECHO learning session to rapidly disseminate findings about Post-COVID Conditions (PCC) and Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS). The series offers presentations by subject matter experts, examples of emerging best practices and models of care, and a facilitated Q&A. This program is intended primarily for providers who care for patients with PCC and ME/CFS but is open to all healthcare professionals and all Long COVID and ME/CFS patient-lived experience experts interested in learning more about the treatment of Long COVID and ME/CFS. CDC's COVIDVaxView Dashboard. The Centers for Disease Control and Prevention (CDC) continues to share weekly COVID-19 vaccination data using a variety of data sources. A new resource, COVIDVaxView Interactive, goes deeper with a look at variations in vaccine confidence by demographics and geography. Home Test to Treat is a new program funded by the National Institutes of Health focused on providing COVID-19 care and treatment for adults who are at greater risk of being affected by COVID-19. Adults who do not currently have COVID-19 or flu may enroll and receive free tests if they are uninsured or are enrolled in Medicare, Medicaid, the Veterans Affairs health care system, or the Indian Health Services. Individuals who are currently positive for COVID-19 can enroll to receive telehealth services for COVID-19 and treatment (if prescribed) at no cost. Enrollees may also be invited to participate in a completely optional research component of the program. Informational flyers are available in English and Spanish. Mobilizing Health Care Workforce via Telehealth. ProviderBridge.org was created through the Coronavirus Aid, Relief, and Economic Security (CARES) Act by the Federation of State Medical Boards and HRSA's Office for the Advancement of Telehealth. The site provides up-to-date information on emergency regulation and licensing by state as well as a provider portal to connect volunteer health care professionals to state agencies and health care entities. Online Resource for Licensure of Occupational Therapists, Physical Therapists, Psychologists, and Social Workers. The site provides up-to-date information on emergency regulation and licensing in each state. Funding and Opportunities First Nations Development Institute Native Farm to School Training and Technical Assistance – April 30. The national nonprofit has a $500,000 grant from the U.S. Department of Agriculture for a youth-centered food sovereignty project under First Nations Development Institute's Nourishing Native Foods and Health Program. The funding will provide training and technical assistance to program coordinators, school staff, teachers, and community organizers in Native communities that are looking to create or expand farm-to-school programs, enhance education curriculum, and/or engage new supply chain partners for providing nutritious school meals and nutrition education. AHRQ Safety Program for Telemedicine: Improving Antibiotic Use – May 23. The Agency for Healthcare Research and Quality (AHRQ) is recruiting practices for a no-cost program beginning in June 2024 to promote appropriate antibiotic use while maintaining patient satisfaction and reducing potential side effects in patients seen via telemedicine. Practices will receive training and one-on-one coaching to improve in the diagnosis and treatment of infections in the telemedicine environment. CMEs, CEUs, and ABIM MOC points are available. Administration for Children and Families – Next Generation Child Support Employment Services Demonstration – June 3. This division of the Department of Health & Human Services modified certain requirements for its program that funds child support services for noncustodial parents. The change is in response to consistent feedback from tribes about the hardship that cost-sharing – that is, the amount of funding a grantee must provide in addition to federal funds – for tribes with existing child support programs and those considering a program. All state and tribal child support programs are eligible for this funding intended to enhance employment services related to child support. CMS Minority Research Grant Program – June 3. The Centers for Medicare & Medicaid Services (CMS) will award up to five grants totaling up to $1,275,000 to support researchers at minority-serving institutions who are investigating or addressing health care disparities affecting all minority populations, including racial and ethnic minority groups, people with disabilities, members of the lesbian, gay, bisexual, transgender, and queer (LGBTQ+) community, individuals with limited English proficiency, individuals residing in rural areas, and individuals adversely affected by persistent poverty or inequality. SAMHSA Minority Fellowship Program – June 10. The Substance Abuse and Mental Health Services Administration (SAMHSA) will invest $15.4 million to recruit, train, and support master's and doctoral-level students in behavioral health care professions to address disparities for racial and ethnic minority populations. Eligibility is limited to nationally representative mental health and/or substance use disorder treatment professional organizations within eight professional fields: psychiatry, nursing, social work, psychology, marriage and family therapy, mental health counseling, substance use disorder and addictions counseling, and addiction medicine. Department of Labor Workforce Opportunity for Rural Communities (WORC) for Appalachian, Delta, and Northern Border Regions – June 20. The DOL collaborates with federal Appalachian and Delta Regional Authorities for grants that will support career, training, and support services in high-demand industries that include manufacturing, IT, health care, and energy. FTA Seeking Comments on Updates to Rural Areas Formula Grant Programs – Comment by June 3. The Federal Transit Administration (FTA) is seeking input on proposed updates to its Rural Areas Formula Grant Programs Circular, which updates FTA guidance for formula grants under the Formula Grants for Rural Areas, the rural component of the Grants for Buses and Bus Facilities, and the Tribal Transit program. The proposed changes are intended to support recipients so they may successfully implement and administer these programs and are a result of changes in the law since the last updates took place. The Formula Grants for Rural Areas program provides capital, planning, and operating assistance to states to support public transportation in rural areas with populations of less than 50,000, where many residents often rely on public transit to reach their destinations. The program also provides funding for state and national training and technical assistance through the Rural Transportation Assistance Program. Proposed Updates to Medicare Hospital Payment for Fiscal Year 2025 – Comment by June 10. The proposed rule from the Centers for Medicare & Medicaid Services (CMS) seeks public comment on payment updates and policy changes to Medicare's inpatient and long-term care hospital prospective payment system (IPPS). For fiscal year 2025, CMS proposes a 2.6 percent increase to acute care hospitals that successfully participate in the Inpatient Quality Reporting program and are meaningful electronic health record users. They also propose to provide a separate payment to small, independent IPPS hospitals to establish and maintain a buffer stock of essential medicines for use during future shortages; to distribute an additional 200 Medicare-funded residency positions (or "slots") to train physicians as required by statute; to adjust hospital payments when inadequate housing and housing instability is indicated; and to extend certain COVID-19 and influenza data reporting requirements for hospitals and Critical Access Hospitals (CAHs). Additional proposals and requests for information that rural stakeholders may want to provide feedback on include: - A Mandatory CMMI Episode-Based Payment Model – The Centers for Medicare & Medicaid Services Innovation Center (CMMI) is proposing a new, mandatory alternative payment model, Transforming Episode Accountability Model (TEAM). For this model, acute care hospitals in select geographic areas would be accountable for ensuring that Medicare beneficiaries receive coordinated, high-quality care during and after certain surgical procedures: coronary artery bypass graft (CABG), lower extremity joint replacement (LEJR), major bowel procedure, surgical hip/femur fracture treatment (SHFFT), and spinal fusion. TEAM would test whether financial accountability for these episode categories reduces Medicare expenditures while preserving or enhancing the quality of care for Medicare beneficiaries. Additionally, within TEAM, CMS proposes a voluntary Decarbonization and Resilience Initiative to assist hospitals in addressing the effects of hospital carbon emissions on health outcomes, health care costs, and quality of care.
- Modifications to the Criteria for New Residency Programs - Currently, to be considered a "new" residency program, a non-teaching hospital must ensure that the residents, the program director, and the teaching staff are all new. To clarify what is meant by "new", CMS proposes that at least 90 percent of the individual resident trainees must not have previous training in the same specialty as the new program. As small or rural-based programs developing new residencies may have difficulty meeting this threshold, CMS is soliciting comments on what should be considered a "small" program, i.e., 16 or fewer resident positions, and what percentage threshold or other approach regarding new resident trainees should be applied to these programs. Additionally, CMS seeks input on a reasonable threshold for determining the newness for faculty and Program Directors as well as whether there should be different thresholds for small residency programs, which may include rural.
- A Targeted Obstetrical Services Condition of Participation (CoP) for Hospitals, CAHs, and Rural Emergency Hospitals (REHs) - CMS plans to propose baseline health and safety standards for obstetrical services in the calendar year (CY) 2025 Outpatient Prospective Payment System/Ambulatory Surgical Center (ASC) proposed rule, which will be published in a few months. To establish those standards, CMS seeks public comment on what the overarching requirement, scope, and structure for an obstetrical services CoP should be and what types of facilities and care settings should such a CoP apply to (that is, all hospitals, hospitals with/without OB units, hospitals with/without emergency services, CAHs, REHs, or outpatient settings).
CMS Updates to the Low-Income Beneficiaries through Limited Income Newly Eligible Transition Program – Comment by June 11. The Centers for Medicare & Medicaid Services (CMS) seeks public comment on the following information collections: 1) Transitional Coverage and Retroactive Medicare Part D Coverage for Certain Low-Income Beneficiaries through the Limited Income Newly Eligible Transition Program; and 2) Appointment of Representative and Supporting Regulations in 42 CFR 405.910, which says that an individual or entity may appoint a representative to act of their behalf when exercising their rights for claim determinations and appeals. According to a March 2024 brief from analysts at the U.S. Department of Health and Human Services, nearly 8 million Part D enrollee reside in rural areas. In addition to higher rates of illness and disease compared to urban enrollees, rural beneficiaries report greater health care costs-related problems and difficulty accessing affordable prescription drugs.
