What's New

CMS Announces First Rural Hackathon. The Centers for Medicare & Medicaid Services (CMS) Innovation Center announced on June 22 that it will conduct the 2024 Rural Health Hackathon, a three-day series of in-person, collaborative sessions designed to generate and construct creative and actionable ideas to address rural health challenges. The solution-oriented events will bring together rural health community care providers, community organizations, industry and tech entrepreneurs, funders, policy experts, and beneficiaries to leverage the wisdom and experience of the crowd and generate new ideas to address some of the top challenges impacting health care in rural health. The following locations will host this year's events:

  • Bozeman, Montana: August 14
  • Dallas, Texas: August 22
  • Wilson, North Carolina: August 29

Individuals interested in attending an in-person session should complete the five-minute Interest Survey by 11:59 pm Eastern on July 12.

New Funding: HRSA Delta Region Maternal Care Coordination Program – Apply by August 2.  HRSA's Federal Office of Rural Health Policy (FORHP) expects to award up to four cooperative agreements for a total of approximately $7 million over four years.  Eligible applicants are public, private, or nonprofit organizations located within the parishes and counties served by the federal Delta Regional Authority.  Program funding is focused on strengthening maternal care and reducing disparities in the Delta region by expanding access and coordination of health care services before, during, and after pregnancy.  An hour-long technical assistance webinar for applicants will be held on Tuesday, July 9 at 3:00 pm Eastern.

CMS Now Accepting Applications from States for Innovation in Behavioral Health Model – Apply by September 9.  Last week, the Centers for Medicare & Medicaid Services (CMS) released a Notice of Funding Opportunity (NOFO) for the Innovation in Behavioral Health (IBH) Model. The IBH Model is focused on improving the behavioral and physical health outcomes and quality of care for people with Medicaid and Medicare who experience moderate to severe behavioral health conditions. The IBH Model is a state-based model, led by state Medicaid agencies (SMAs), and CMS will select up to eight SMAs to participate in the IBH model via cooperative agreement funding. CMS is hosting a webinar for states interested in applying on Thursday, July 11 at 2:00 pm Eastern.

New Funding: CMS Transforming Maternal Health (TMaH) Model – Apply by September 20.  The TMaH Model's primary focus is improving health outcomes for mothers and their infants who are enrolled in Medicaid and Childrens' Health Insurance Program (CHIP). Under TMaH, selected state Medicaid agencies will receive targeted technical support to develop a whole-person approach to pregnancy, childbirth, and postpartum care.  Medicaid pays for nearly half of all births nationally and covers a greater share of births in rural areas.  Rural residents have a 9 percent greater chance of experiencing severe maternal morbidity and mortality compared with urban residents.  The Centers for Medicare & Medicaid Services (CMS) expects to award cooperative agreements to up to 15 state Medicaid agencies. 

Surgeon General Declares Firearm Violence a Public Health Crisis.  This week, the nation's top health official made the first-ever advisory addressing the issue, citing new data from the Centers for Disease Control and Prevention and declaring firearm violence as an "urgent threat to the health and well-being of our country." Compared with rates of emergency medical service (EMS) encounters in 2019, EMS increased among multiple demographic groups and across all county-level factors. The largest age group-specific increases occurred among children and adolescents aged 0-14 years; for children aged 1-19 years, firearms are now the leading cause of death.  Last year, the nonprofit KFF used federal data to report that firearms contributed to a record-high number of suicide deaths in 2022.  Rates were highest among American Indian and Alaska Native (AIAN) people, males, and people who live in rural areas.

Updated Fact Sheets from ERS on Rural Economy.  The Economic Research Service (ERS) at the U.S. Department of Agriculture provides the latest on population, income, poverty, food security, education, and more in State Fact Sheets.  County-level poverty rates, population change, educational attainment, and unemployment rates/median household income have been updated in County-Level Data Sets

Ongoing: Requirements for Buprenorphine PrescribingAs announced by the Substance Abuse and Mental Health Services Administration in January 2023, clinicians no longer need a federal waiver to prescribe buprenorphine for treatment of opioid use disorder.  Clinicians are still required to register with the federal Drug Enforcement Agency (DEA) to prescribe controlled medications.  On June 27, 2023, the DEA began to require that registration applicants – both new and renewing – affirm they have completed a new, one-time, eight-hour training.  Exceptions for the new training requirement are practitioners who are board certified in addiction medicine or addiction psychiatry, and those who graduated from a medical, dental, physician assistant, or advanced practice nursing school in the U.S. within five years of June 27, 2023.  Watch this 11-minute video that explains the changes.  Rural Health Clinics (RHCs) still have the opportunity to apply for a $3,000 payment on behalf of each provider who trained between January 1, 2019 and December 29, 2022 (when Congress eliminated the waiver requirement).  Approximately $889,000 in program funding remains available for RHCs and will be paid on a first-come, first-served basis until funds are exhausted.  Send questions to DATA2000WaiverPayments@hrsa.gov.


COVID-19 Resources

Learn About Long COVID & Fatiguing Illness  – Thursday, July 11 at 3:00 pm Eastern.  The Centers for Disease Control and Prevention funds this monthly webinar-style ECHO learning session to rapidly disseminate findings about Post-COVID Conditions (PCC) and Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS). The series offers presentations by subject matter experts, examples of emerging best practices and models of care, and a facilitated Q&A. This program is intended primarily for providers who care for patients with PCC and ME/CFS but is open to all healthcare professionals and all Long COVID and ME/CFS patient-lived experience experts interested in learning more about the treatment of Long COVID and ME/CFS.

Mobilizing Health Care Workforce via Telehealth.  ProviderBridge.org was created through the Coronavirus Aid, Relief, and Economic Security (CARES) Act by the Federation of State Medical Boards and HRSA's Office for the Advancement of Telehealth. The site provides up-to-date information on emergency regulation and licensing by state as well as a provider portal to connect volunteer health care professionals to state agencies and health care entities. 


Funding and Opportunities

DOJ National Sexual Assault Kit Initiative – July 29.  The U.S. Department of Justice (DOJ) is planning 29 awards, each up to $2.5 million to support criminal investigations with sexual assault kits that have not been submitted to a forensic laboratory for testing, as well as other violent cold case crimes.  Eligible applicants are federally recognized Native American tribal governments and small law enforcement agencies that meet certain criteria. 

Foundation for Opioid Response Efforts (FORE) Opioid Crisis Innovation Challenge – August 8.  The national nonprofit encourages application from rural areas for funding that aims to address long-standing and complex issues related to the nation's opioid crisis.  Proposed projects should bring together multi-disciplinary, cross-sector teams to reduce overdoses, increase access to treatment, and support long-term recovery.

DHS Funding One Entity for Rural Emergency Medical Communications Demonstration Project – August 9.  The U.S. Department of Homeland Security (DHS) provides funding for a single entity to enhance existing emergency communications infrastructure to improve delivery of rural medical care and address implementation gaps in the National Emergency Communications Plan.

AHRQ TeamSTEPPS®: No Cost Training for Patient Safety – Sessions through November 2024.  The federal Agency for Healthcare Research and Quality (AHRQ) offers virtual and limited in-person trainings led by experienced facilitators at no charge to participating health care teams and individuals.  The updated Team Strategies & Tools to Enhance Performance & Patient Safety (TeamSTEPPS®) training includes a course to support Diagnosis Improvement, where multidisciplinary teams of three or more members engaged in diagnostic decision making can participate and earn continuing education units.

New Research on Rural Hospital Profitability from 2018 to 2023. Profitability of rural hospitals in 2020-21 and 2021-22 was influenced by the Public Health Emergency (PHE) funding distributed during the COVID-19 pandemic. Three briefs from the North Carolina Rural Health Research and Policy Analysis Center examine hospital profitability from 2018 to 2023, taking PHE funding into account.

HHS Finalizes Final Rule Establishing Disincentives for Health Care Providers Who Have Committed Information Blocking. The Department of Health and Human Services (HHS) agencies, including the Office of the National Coordinator for Health Information Technology (ONC), the Centers for Medicare & Medicaid Services (CMS), the Office of Inspector General (OIG) released the 21st Century Cures Act: Establishment of Disincentives for Health Care Providers That Have Committed Information Blocking (Disincentives Final Rule). Information blocking by healthcare providers is when they engage in practices they knew were unreasonable and likely to interfere with, prevent, or materially discourage the access, exchange, or use of electronic health information (EHI), except as required by law or covered by a regulatory exception. This final rule establishes disincentives for Medicare-enrolled providers determined by the OIG to have engaged in information blocking.  Specifically, under the Medicare Promoting Interoperability Program, hospitals found to have committed information blocking would no longer be a meaningful user, so they would not receive the program's positive payment adjustment. Critical Access Hospitals would have their payments reduced from 101% to 100% of reasonable costs, while clinicians in Medicare's Merit-based Incentive Payment System (MIPS) would receive a score of zero in the MIPS Promoting Interoperability performance category. Under the Medicare Shared Savings Program (SSP), accountable care organizations (ACOs) or providers participating in the ACO that commit information blocking may be ineligible to participate in SSP for at least one year and may not receive revenue they may have earned through the program. ONC and CMS released a fact sheet and Frequently Asked Questions which provide an overview of select provisions of the final rule.  

Deadline for Disputes Under No Surprises Act ExtendedThe Departments of Health and Human Services, Labor, and the Treasury announced that providers, facilities, and providers of air ambulance services whose ability to timely initiate open negotiation under the No Surprises Act was impacted by the Change Healthcare cybersecurity incident in February may initiate negotiations for independent dispute resolutions at any point between 6/14/2024 and 10/12/2024, regardless of when the payment or notice of denial of payment and disclosures were transmitted. To take advantage of this exception period, providers must furnish an attestation that their ability to initiate a timely open negotiation for an item or service was impacted by the effects of the cybersecurity incident alongside the standard open negotiation initiation form.

MedPAC June Report to Congress Highlights Rural Considerations for Medicare Advantage Provider Networks. As part of its mandate from Congress, the Medicare Payment Advisory Commission (MedPAC) reports each June on improvements to Medicare payment systems and issues affecting the Medicare program, including changes to health care delivery and the market for health care services. This year's report addresses approaches for updating clinician payments and incentivizing participation in alternative payment models, rural considerations for network adequacy standards and prior authorization in Medicare Advantage, assessing health care utilization data sources for Medicare Advantage enrollees, paying for software technologies in Medicare, and Medicare's Acute Hospital Care at Home program.

Call for nominations for a CMS Maternal Health Technical Expert Panel – Submit by July 12. The Centers for Medicare & Medicaid Services (CMS) seeks nominations for a Maternal Health Technical Expert Panel (TEP) of approximately 20 individuals who will provide input on the expansion of the Maternal Morbidity Structural Measure (MMSM) and expansion and scoring methodology for and awarding of the Birthing-Friendly Hospital Designation. The MMSM assesses whether or not a hospital participates in a Statewide or National Perinatal Quality Improvement Collaborative initiative and implements patient safety practices and/or bundles related to maternal morbidity. The Birthing-Friendly Hospital Designation is currently awarded to hospitals and health systems based on their positive response to the MMSM.  CMS seeks nominations from individuals with differing areas of expertise and perspectives, including clinical experts in maternal care, chairs of obstetrical/gynecologic departments, hospital administrators, perinatal quality improvement experts, health equity and birth justice experts, statistical and performance measurement experts, and consumer/patient/family (caregiver) experts.  Send questions to the Maternal Quality inbox at cmsmaternalquality@yale.edu.

Call for Nominations to the Physician-Focused Payment Model Technical Advisory Committee – Submit by July 10.  The Government Accountability Office (GAO) is accepting nominations for the Physician-Focused Payment Model Technical Advisory Committee (PTAC).  This independent federal advisory committee consists of eleven members from a wide-range of backgrounds who make recommendations to the Secretary of Health and Human Services (HHS) on stakeholder-submitted physician-focused payment models and related topics. The group is supported by staff from the Office of the Assistant Secretary for Planning and Evaluation (ASPE) within HHS.  Last year, the group held meetings in September addressing challenges facing patients and providers in rural communities, approaches for incorporating rural providers in provider-based total cost of care (PB-TCOC) model design, provider perspectives on payment issues related to rural providers in PB-TCOC models, incentives for increasing rural providers' participation in PB-TCOC models, and successful interventions and models for encouraging value-based transformation in rural areas. 

HHS Finalizes Section 1557 Nondiscrimination Rule – Impacts for RHCs – Monday, July 15 at 2:00 pm Eastern. The National Association of Rural Health Clinics (NARHC) will host the free, FORHP-supported webinar, with information on the Department of Health and Human Services (HHS) Office of Civil Rights and the Centers for Medicare & Medicaid Services final rule under Section 1557 of the Affordable Care Act Nondiscrimination in Health Programs and Activities and the RHC impacts, including: 

  • Requirement to post a notice of nondiscrimination.
  • Requirements to post notice of availability of translation and auxiliary aid services in top 15 foreign languages.
  • Requirement to establish and train employees on policies and procedures related to nondiscrimination.

Additional time for Q&A will be provided. Advanced registration is required.

ACO PC Flex Model Office Hour – Tuesday, July 16 at 2:00 pm Eastern. In this hour-long session, the Centers for Medicare and Medicaid Innovation will provide an overview of the new ACO Primary Care Flex Model (ACO PC Flex Model), which will focus on primary care delivery in the Medicare Shared Savings Program (SSP).  CMS has released a Request for Applications  and the application portal is open until August 1 for new or renewing ACOs who submitted an application to SSP by June 17, 2024.  CMS plans to announce applicants selected to participate in October 2024.  This model seeks to increase accountable care relationships for people with Medicare, especially those in rural and underserved communities.

Resource of the Week

Is Rural Emergency Hospital the Best Option for You?  The FORHP-funded Rural Emergency Hospital Technical Assistance Center (the Rural Health Redesign Center) provides a recording of the June 6 webinar.  Technical experts discuss a new federal designation, the Rural Emergency Hospital, a provider type designed to address concerns that some rural hospitals would not be able to sustain operations and could be at risk of closure. The Rural Health Redesign Center provides free financial, clinical and operational assessments to facilities considering the REH designation to inform their decision making.   The session includes two CEOs who consulted with the Center and decided to convert, and one CEO who went through the process and decided against converting.

An Introduction to Philanthropy in Rural Aging.  This resource from the nonprofit Grantmakers in Aging offers guidance on private nonprofit grant funds for rural aging work, with a look at the scale of current funding, rural assets, sustainability, and partnership building.  Issues related to rural aging are highlighted, including cultural sensitivities, transportation, broadband, housing, institutional care, and social isolation.
