What's New

New National Strategy for Suicide Prevention.  On Wednesday, the U.S. Department of Health & Human Services released details of a plan for more than 200 actions across the federal government to address high rates of suicide.  Populations in the United States disproportionately impacted by suicide include non-Hispanic American Indian and Alaska Native youth, middle-aged and older adults, non-Hispanic White males, rural populations, and veterans, among others. In particular, rural communities continue to see increases in suicide rates. According to 2023 data from the Centers for Disease Control and Prevention, suicides in non-urban environments increased 46 percent between 2000 and 2020.

HRSA Funding: Delta Health Systems Implementation Program – Apply by June 24.  New funding administered by FORHP supports hospitals in the Mississippi Delta region with projects that will improve their financial sustainability and allow for increased access to care in rural communities. Approximately $2,000,000 across 5 awards will be aimed at financial and operational improvement, quality improvement, telehealth, and workforce development. Interested applicants may attend a live webinar on Wednesday, May 1 at 2:00 pm Eastern. 

NIH Strategies to Improve Health Outcomes/Advance Health Equity in Rural Populations – Apply by May 17.  The National Institutes of Health (NIH) will support research to develop, adapt, or implement intervention strategies addressing social determinants of health.  The Notice of Funding Opportunity notes that many interventions that address SDOH are developed, tested, and implemented only in urban areas and encourages a wide range of methodological approaches to innovate research in rural areas, such as community-engaged research.

Missouri Medicaid Tests New Transformation of Rural Community Health (ToRCH) Model. The new program aims to address social determinants of health and will have hospitals serve as community-based hubs, or regional leads, to direct strategy and coordinate the efforts of health care providers, community-based organizations (CBOs), and social service agencies within a designated rural community. Medicaid funds will support hub activities, such as leadership, data analysis, and management of community partnerships, and community partners may request funds to pay for additional space, purchase IT or other equipment, purchase a vehicle, or obtain new training for staff.  Additional ToRCH funds will be available to reimburse CBO partners for approved health related social needs services identified in the ToRCH communities, such as supplemental health-related transportation, food and nutrition education, home-delivered medically appropriate meals, and housing remediation to address health risks, on a per-person, per-service basis.  The first cohort of six hospitals will begin on July 1 of this year.

Webinar on the Role of Primary Care in the AHEAD Model – Thursday, May 9 at 1:00 pm Eastern.  During this webinar, the Centers for Medicare & Medicaid Services (CMS) will provide more about the new CMS Innovation Center model States Advancing All-Payer Health Equity Approaches and Development (AHEAD) Model. Topics will include provider eligibility, opportunities for participating State Medicaid Agencies to boost participation and increase statewide primary care investment, and the relationship between the hospital and primary care components of the AHEAD Model.  CMS encourages participants to submit questions to the AHEAD mailbox (AHEAD@cms.hhs.gov) in advance of this webinar.

Ongoing: Requirements for Buprenorphine PrescribingAs announced by the Substance Abuse and Mental Health Services Administration in January 2023, clinicians no longer need a federal waiver to prescribe buprenorphine for treatment of opioid use disorder.  Clinicians are still required to register with the federal Drug Enforcement Agency (DEA) to prescribe controlled medications.  On June 27, 2023, the DEA began to require that registration applicants – both new and renewing – affirm they have completed a new, one-time, eight-hour training.  Exceptions for the new training requirement are practitioners who are board certified in addiction medicine or addiction psychiatry, and those who graduated from a medical, dental, physician assistant, or advanced practice nursing school in the U.S. within five years of June 27, 2023.  Watch this 11-minute video that explains the changes.  Rural Health Clinics (RHCs) still have the opportunity to apply for a $3,000 payment on behalf of each provider who trained between January 1, 2019 and December 29, 2022 (when Congress eliminated the waiver requirement).  Approximately $889,000 in program funding remains available for RHCs and will be paid on a first-come, first-served basis until funds are exhausted.  Send questions to DATA2000WaiverPayments@hrsa.gov.


COVID-19 Resources

Learn About Long COVID & Fatiguing Illness  – Thursday, May 9 at 3:00 pm Eastern.  The Centers for Disease Control and Prevention funds this monthly webinar-style ECHO learning session to rapidly disseminate findings about Post-COVID Conditions (PCC) and Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS). The series offers presentations by subject matter experts, examples of emerging best practices and models of care, and a facilitated Q&A. This program is intended primarily for providers who care for patients with PCC and ME/CFS but is open to all healthcare professionals and all Long COVID and ME/CFS patient-lived experience experts interested in learning more about the treatment of Long COVID and ME/CFS.

CDC's COVIDVaxView Dashboard.  The Centers for Disease Control and Prevention (CDC) continues to share weekly COVID-19 vaccination data using a variety of data sources.  A new resource, COVIDVaxView Interactive, goes deeper with a look at variations in vaccine confidence by demographics and geography. 

Home Test to Treat is a new program funded by the National Institutes of Health focused on providing COVID-19 care and treatment for adults who are at greater risk of being affected by COVID-19.  Adults who do not currently have COVID-19 or flu may enroll and receive free tests if they are uninsured or are enrolled in Medicare, Medicaid, the Veterans Affairs health care system, or the Indian Health Services. Individuals who are currently positive for COVID-19 can enroll to receive telehealth services for COVID-19 and treatment (if prescribed) at no cost. Enrollees may also be invited to participate in a completely optional research component of the program. Informational flyers are available in English and Spanish.

Mobilizing Health Care Workforce via Telehealth.  ProviderBridge.org was created through the Coronavirus Aid, Relief, and Economic Security (CARES) Act by the Federation of State Medical Boards and HRSA's Office for the Advancement of Telehealth. The site provides up-to-date information on emergency regulation and licensing by state as well as a provider portal to connect volunteer health care professionals to state agencies and health care entities. 

Online Resource for Licensure of Occupational Therapists, Physical Therapists, Psychologists, and Social Workers.  The site provides up-to-date information on emergency regulation and licensing in each state.


Funding and Opportunities

First Nations Development Institute/Luce Foundation Indigenous Knowledge Fellowship – May 7.   A self-directed fellowship for Native Americans engaged in meaningful work that benefits Indigenous people and communities in reservation and/or urban settings. The fellowship is open to both emerging and experienced leaders from a wide variety of fields, including but not limited to healthcare, agriculture, food systems, youth leadership development, natural resource management, climate change, economic development, journalism, language and cultural revitalization, and traditional or contemporary arts.

Dept of Ed Mental Health Service Professional Demonstration Grants – May 15.  Federal grants ranging from $400,000 to $1 million will support up to 33 awards for a five-year project.  Applicants from high-need school districts must propose a school-based mental health partnership established to place graduate students in mental health fields in local education agencies (LEAs). Rural high need LEAs include agencies that meet eligibility requirements for funding under the Small, Rural School Achievement Program and Rural Low-Income School Program

USDA Circuit Rider Program – Technical Assistance for Rural Water Systems – Ongoing.  The U.S. Department of Agriculture (USDA) accepts applications on an ongoing basis from rural water system operators that need assistance with meeting statutory and regulatory requirements.  Among many issues, technical assistance topics include day-to-day operations, management/finance, operation/maintenance, loan application and reporting, and disaster/emergency assistance.

Research Recap: LGBTQ+ Health: Rural vs Urban Inequities. From 2019-2020, rural lesbian, gay, and bisexual adults were more likely to report poor/fair self-rated health than rural heterosexual adults.  This  Recap summarizes several policy briefs from the University of Minnesota Rural Health Research Center examining self-rated health, chronic conditions, mental health, and social and emotional support systems for LGBTQ+ populations. 

Identifying Rural Health Clinics Within the Transformed Medicaid Statistical Information System (T-MSIS) Analytic Files.  There is limited information on the extent to which Rural Health Clinics (RHC) provide pediatric and pregnancy-related services to individuals enrolled in state Medicaid/CHIP programs. In part, this is because methods to identify RHC encounters within Medicaid claims data are outdated. This brief from the Maine Rural Health Research Center describes a methodology for identifying RHC encounters within the Medicaid claims data using Transformed Medicaid Statistical Information System (T-MSIS) Analytic Files.

CMS Issues Two Final Rules: Ensuring Access to Medicaid Services and Medicaid and CHIP Managed Care Access, Finance, and Quality.  Together, these rules from the Centers for Medicare & Medicaid Services (CMS), advance access to care, access to quality of care and aim to improve health outcomes across Medicaid fee-for-service (FFS) and managed care plans. The Access Rule addresses dimensions of access across both Medicaid FFS and managed care delivery systems, including increasing transparency and accountability, standardizing data and monitoring, and creating opportunities for states to promote beneficiary engagement.  The Managed Care Rule addresses standards for timely access to care and states' monitoring and enforcement efforts; quality and fiscal and program integrity standards for state directed payments; the scope of in lieu of services and settings; establishment of a quality rating system (QRS) for Medicaid and CHIP managed care plans.

340B Drug Pricing Program: Administrative Dispute Resolution Final Rule.  On April 18, the Health Resources and Services Administration released a final rule establishing an Administrative Dispute Resolution (ADR) process for the 340B Drug Pricing Program that is effective on June 18.  The federal 340B program allows certain hospitals and clinics, aka 'covered entities,' that treat low-income and uninsured patients to buy outpatient prescription drugs from manufacturers at a discount, and it is intended to help these providers stretch scarce federal resources as far as possible.  This rule finalizes new requirements and procedures to make the resolution of disputes between covered entities and manufacturers more accessible and efficient. 

CMS Finalizes Minimum Staffing Standards for Nursing Homes. On April 22, 2024, the Centers for Medicare & Medicaid Services (CMS) issued the Minimum Staffing Standards for Long-Term Care (LTC) Facilities and Medicaid Institutional Payment Transparency Reporting final rule. This rule aims to hold nursing homes accountable for providing safe and high-quality care for nearly 1.2 million residents living in Medicare and Medicaid-certified long term care facilities. Central to the rule are comprehensive minimum nurse staffing standard requirements, which aim to significantly reduce the risk of residents receiving unsafe and low-quality care within LTC facilities.  As the long-term care sector continues to recover from the COVID-19 pandemic, the final standards also consider the local realities in rural and underserved communities via staggered implementation and exemptions processes. Highlights of finalized policies include a total nurse staffing standard of 3.48 hours per resident day (HPRD), which must include at least 0.55 HPRD of direct registered nurse (RN) care and 2.45 HPRD of direct nurse aide care. CMS also finalizing enhanced facility assessment requirements and a requirement to have an RN onsite 24 hours a day, seven days a week, to provide skilled nursing care. Finally, CMS finalizing to define "rural" in accordance with the Office of Management and Budget (OMB) definition. OMB designates counties as Metropolitan (metro), Micropolitan (micro), or neither. "A Metro area contains a core urban area of 50,000 or more population, and a Micro area contains an urban core of at least 10,000 (but less than 50,000) population. All counties that are not part of a Metropolitan Statistical Area (MSA) are considered rural". As of 2022 the Rural Health Research & Policy Centers Nursing Home Chartbook, 87 percent of non-core counties have dually/Medicaid certified nursing homes while 82 percent of non-core counties have dually/Medicare-certified nursing homes.

New Medicare Behavioral Health Providers: Incorporating MHCs and MFTs Into Your RHC – Thursday, May 9 at 1:00 pm Eastern. The National Association of Rural Health Clinics (NARHC) will host the free, FORHP-supported webinar with information on incorporating Marriage and Family Therapists (MFTs) and Mental Health Counselors (MHCs) into Rural Health Clinics (RHCs). Medicare coverage of Marriage and Family Therapists and Mental Health Counselors in RHCs began January 1, 2024. This webinar will feature representatives from the National Board for Certified Counselors and the American Association for Marriage and Family Therapy, who will provide information to ensure RHCs are equipped to consider expanding behavioral health services. Additional time for Q&A will be provided. Advanced registration is required.


Learning Events and Technical Assistance

SAMHSA: Peer Recovery Support for Families – Thursday, May 2 at 1:00 pm Eastern.  The National Center on Substance Abuse and Child Welfare (NCSACW) holds a one-hour session with experts who will explain how these programs work and how to start one in your community.  The NCSACW is a resource and technical assistance center supported by the Administration for Children and Families and the Substance Abuse and Mental Health Services Administration (SAMHSA).

Rural Center of Excellence for Opioid Response:  Addiction in a Chronic Disorder – Thursday, May 2 at 2:00 pm Eastern.  The Fletcher Group is one of three FORHP-supported Rural Centers of Excellence on Substance Use Disorder.  In this session, experts will discuss extensive models of chronic disease management. 

Nursing Funding Opportunities from HRSA – Wednesday, May 8 at 2:30 pm Eastern.  HRSA's Bureau of Health Workforce will discuss funding opportunities for nursing students and graduates, nurses looking to advance their careers, and nursing education and training programs.  The two-hour webinar will provide information about how to apply for scholarships, nursing student loan and loan repayment opportunities, training programs, and the benefits of each program.  Program representatives will be available to answer questions.


Resources of the Week

Federal Low Income Home Energy Assistance Program.  Also known as LIHEAP, the program provides federally funded assistance to reduce the costs of home energy bills, energy crises, weatherization, and post-disaster needs.  In fiscal year 2023, the Administration for Children and Families (ACF) invested $4 billion for grantees in each state and in five territories, with set-aside for tribal assistance.  In turn, these grantees assist households with low incomes, particularly those that have a high home energy burden and/or have members who are elderly, disabled, and/or a young child.  Any community-based organization can use ACF's Cooling Season Outreach toolkit with fliers, videos, and social media content in several languages.  There's also a LIHEAP search tool that individuals can use to find help.

HeatRisk Forecast Tool.  A color-coded map of the U.S. provides a seven-day heat forecast that indicates where temperatures may reach levels that could harm health.  It's a collaboration between the National Oceanic and Atmospheric Administration and the Centers for Disease Control and Prevention (CDC).  As part of this effort, the CDC also provides a HeatRisk Dashboard with resources for high-heat days, details on local air quality, and actions to stay safe in these conditions.  Finally, the CDC has a site with clinical guidance for heat health, focused initially on treating children with asthma, pregnant women, and people with cardiovascular disease.
